Job Description:
A company is looking for a Telehealth Provider (PA, CNS,
NP).
Key Responsibilities
Conduct telephonic annual wellness visits
Identify patient needs, provide health education, and offer
lifestyle recommendations
Create and review patient care plans for Chronic Care
Management
Required Qualifications
Licensure/certification as a CNS, NP, or PA in Florida, Texas, or
Georgia
Compliance with CEU and licensure requirements in all states where
licensed
Experience in telehealth, wellness visits, and preventative
care
Familiarity with electronic health record (EHR) systems and medical
software
Willingness to obtain licensure in additional states as needed
