Ypsilanti, Michigan, or in the comfort of their own homes. Our

travelThe Behavioral Health Specialist provides essential
therapeutic care to participants in the Huron Valley PACE program.
Youll independently assess participants' behavioral health needs,
coordinate care, and implement targeted interventions. Working
closely with participants and their families/caregivers, youll play
a key role in delivering comprehensive behavioral health services
to support overall well-being, all while collaborating with an
interdisciplinary healthcare team.What youll do:Provide therapeutic
interventions to participants, either on-site at Huron Valley PACE
or in their residences.Actively contribute as a member of the
Interdisciplinary Team (IDT) by attending staff meetings and
participating in care planning.Utilize DICE Case Studies to develop
effective interventions for participants experiencing dementia and
behavioral challenges.Develop and implement individualized
treatment plans, incorporating individual and/or group
psychotherapy, consultations with Primary Care Providers and other
disciplines, crisis intervention, and referrals for psychiatric
services.Assess participant progress toward care plan goals and
communicate updates with the IDT.Maintain accurate and timely
documentation in electronic medical records.Foster a collaborative
and supportive team environment, promoting strong working
relationships among staff.Educate staff on the social,
environmental, emotional, and cultural factors influencing health
and medical care, as well as available community resources.Monitor
and document changes in participants' conditions, ensuring timely
communication with the IDT and family members.Uphold and promote
the Participant Bill of Rights, prioritizing participant
satisfaction and well-being.Adapt to evolving responsibilities as
needed to support the programs mission and participant care.Ability
to conduct home visits and provide care in various settings
(nursing homes, assisted living, adult foster care) with the option
of Telehealth.Skills and Knowledge:Proficiency in reading, writing,
speaking, and understanding English, with the ability to follow
both written and verbal instructions.Patience, tact, positivity,
and enthusiasm in working with participants with varying cognitive
abilities.Expertise in human behavior principles, individual and
group therapy techniques (preferably CBT and DBT), short-term
interventions, substance abuse assessment and treatment, and
relevant regulations.Strong individual therapy skills for geriatric
participants, complemented by excellent analytical thinking and
problem-solving abilities.What it takes:Masters degree from an
accredited School of Social Work or equivalent (LPC or
LLPC).Licensed Professional Counselor (LPC) in Michigan, or Limited
Licensed Professional Counselor (LLPC) under LPC
supervision.Licensed Master Social Worker (LMSW) in Michigan, or
Limited Licensed Master Social Worker (LLMSW) under LMSW
supervision.Minimum of two years of therapy experience.Valid
Michigan drivers license and access to a reliable vehicle.CPR
certification or willingness to obtain.Strong knowledge of
behavioral principles, therapy techniques (preferably CBT and DBT),
and short-term interventions.Experience in substance abuse
assessment and treatment, or willingness to receive
training.Proficiency in providing therapy to geriatric
participants, with strong analytical and problem-solving
skills.Medically cleared for communicable diseases and up-to-date
immunizations before direct participant contact.The above is a
